OBTAIN REPEAT PRESCRIPTIONS?
Repeat prescriptions will be
issued at the doctor's discretion and are normally for patients on long-term
treatment. Requests for prescriptions can be made by placing the repeat
prescription request in the box opposite the pharmacy in the entrance of the health centre or by telephoning
484051 and leaving a message on our 24 hour answering machine.
Where
possible please quote your personal computer number, name and the items
required as shown on your repeat prescription slip. Alternatively, you
may call at the surgery and place your order in the box provided, or
write enclosing a SAE. We are sorry we are unable to issue repeat prescriptions
at weekends, public holidays or out of hours.
Please allow two
working days before collection (or three if prescriptions go direct
to a chemist) and make allowances for weekends or public holidays.
HOW
DO I...
REGISTER WITH THE PRACTICE?
An FP1 form and
two questionnaires are completed and passed in to reception to register
with the Practice. The questionnaires are to give the GP information
about your medical history prior to receiving your medical records from
your previous GP. New baby registration forms should be completed and
passed in at reception as soon as possible after the birth and a computer
record will be created.
Temporary residents to the area may
register for a short or long-stay period, ie up to 15 days or three
months. Patients from non-reciprocal countries may register as private
patients. They will be expected to pay for each consultation, and the
cost of a private prescription etc when necessary. Please ask at reception
for a list of the current fees.
Patients register with the Practice
rather than an individual GP. However, patients have the right to express
a preference to be seen by the practitioner of their choice for the
purposes of continuing care, care of particular conditions, gender or
ethnicity. Where a patient wishes to exercise this right:
(i)
the patient may have to wait longer to see their preferred practitioner.
(ii)
the practitioner has the right of reasonable refusal (eg in respect
of a violent patient).
(iii) the patient may be asked to accept
an alternative if the service required is being delivered by another
member of the practice (eg practice nurse).
